Proxy access

What is proxy access?

Proxy access allows someone else for example; a parent, carer, or family member to access online services on your behalf, using their own online account login. Depending on the level of access agreed, a proxy user may be able to:

  • Book, view, or cancel appointments for you
  • Order repeat prescriptions on your behalf
  • View parts of your medical record online

Proxy access is different from someone simply attending an appointment with you or calling on your behalf; it gives them their own ongoing, secure online access linked to your record, at the level you (or your representative) agree to.

Who can have proxy access?

Proxy access may be appropriate for:

  • Parents or guardians managing a child's health record
  • Carers supporting a patient who is unable to manage their own online access
  • Family members supporting a patient with their consent

Access is only granted with the consent of the patient (where they have capacity to give it), or in line with legal authority to act on their behalf (for example, power of attorney) where they do not.

How to apply

Any patient or carer wishing to have proxy access will need to sign up to this service by completing a form, available at the Practice reception. You will be asked to provide proof of identity, and, depending on the circumstances, evidence of your relationship to the patient or your legal authority to act on their behalf.

We will discuss with you what level of access is appropriate, and this can be reviewed or changed at any time.
